How to Read a Biotech Pipeline as an Investor or Partner

How to Read a Biotech Pipeline as an Investor or Partner

How to Read a Biotech Pipeline as an Investor or Partner

A pipeline chart is a status report, not a scoreboard. Every stage label on it marks a specific regulatory or experimental milestone that a program has actually reached, not the amount of time or money a company has spent trying to get there. Two programs can sit in the same "Phase 1" column and mean very different things depending on what data backs that label, who is paying for the next step, and how recently the chart was updated.

For investors and business development readers, the real skill isn't memorizing stage names. It's knowing what question each stage should raise, what a chart typically hides, and how to go find the missing piece before you commit capital or a deal term to it. That work runs in a practical order, from gathering the right source documents to sizing the risk on a specific program.

Gather the Pipeline Materials Before You Start

Before you try to interpret a pipeline chart, pull together the documents that sit behind it. You want the company's current pipeline page or investor deck, its most recent 10-K or 10-Q, the matching entries on clinicaltrials.gov for each listed program, and any recent press releases covering IND filings or dosing milestones.

Pipeline graphics are marketing and investor-relations material. They get updated on their own schedule, and that schedule often lags the underlying regulatory filings by weeks or months. A program can advance into a new phase at the FDA before the public chart catches up, or a chart can keep showing a stage the company has already moved past. Check the chart's publish date against the date of the most recent filing or press release for that program. If they don't line up, treat the chart's stage marker as provisional until you confirm it against the primary source.

Learn the Stage Definitions Each Program Sits In

Every program on a pipeline chart sits in one of three broad categories: research, preclinical, or clinical. Each category is defined by a specific milestone reached, not by how long the program has existed, and each one sets up the next.

Research and Preclinical Stage Differences

Research stage covers target identification and lead optimization done in vitro, in cells or in a dish. No IND-enabling studies are underway yet, and no dose or safety data exists because nothing has been given to a living animal model as part of a formal study. A company at this stage is still working out whether its molecule or cell does what it's supposed to do under controlled lab conditions.

Preclinical stage adds the toxicology, pharmacology, and manufacturing studies the FDA requires before it will accept an IND, or Investigational New Drug, application. This is the formal request to the FDA for permission to test a candidate in people. The spending pattern reflects that shift: average spend on early discovery before venture capital typically gets involved runs near $10 million, while the preclinical phase itself averages $3 million to $25 million, largely because of the animal toxicology and manufacturing scale-up work required. The IND filing itself is the transition point. A program that has completed its IND-enabling studies and filed is functionally done with preclinical work and waiting on the FDA's response.

Phase I through Phase IV Clinical Trial Stages

Once the FDA clears an IND, a program enters human testing, structured in four phases. Phase I tests safety in a small group of participants, usually looking for a tolerable dose range and any immediate adverse effects. Phase II expands the group and starts testing efficacy and dosing together, trying to find a dose that actually works without unacceptable side effects. Phase III confirms efficacy at a much larger scale, often across multiple sites, and is the data set the FDA uses to decide on approval. Phase IV covers post-approval surveillance, monitoring an approved therapy in real-world use for effects that a smaller trial population wouldn't have surfaced.

Attrition at each step is steep. Only about 30% of Phase II candidates ever reach a Phase III trial, which makes the Phase II to Phase III transition one of the sharpest filters in the whole pipeline. Compounds that do reach Phase III succeed just under half the time historically. A failed trial is not a minor setback financially: it costs the sponsor somewhere between $800 million and $1.4 billion in sunk development spend, and the full path from IND to FDA approval averages nine to fifteen years. Those numbers are the backdrop against which you should read any clinical-stage entry on a chart.

Cell therapy programs carry the same Phase I through Phase IV labels, but the trial design underneath them often looks different from a small-molecule drug. Ascending-dose cohort studies are common: a first group of patients is dosed at a lower level, monitored for a defined safety window, and only after that window closes does the next cohort receive a higher dose. SMSbiotech's own regulatory path with its investigational nebulized SMS Cells program in COPD illustrates how that terminology maps onto a chart. An FDA "Study May Proceed" notification following IND review is what moves a program into its Phase 1b stage, and each ascending-dose cohort in that kind of trial completes its own defined monitoring period before the next cohort begins dosing. None of that speaks to whether a given therapy works or is safe for any particular patient; it simply tells you what the Phase 1b label on a cell therapy chart actually represents procedurally, which is the information you need to read the stage marker correctly.

Read the Pipeline Chart Itself

Most pipeline charts arrange programs as rows and development stages as columns, or the reverse, with a bar or shaded marker showing how far each program has progressed across those columns. The exact visual convention varies by company, so don't assume one chart's layout transfers to the next company you look at.

What's useful is comparing the same chart across two or more reporting periods. A bar that moves forward tells you the program advanced. A bar that stays flat tells you nothing moved, which on its own might just mean a trial is still enrolling. A program that simply disappears from the chart between two periods usually signals it was discontinued, and that's worth a direct question to management if it isn't addressed elsewhere in the filing. A bar sitting in the exact same position across more than two reporting cycles, with no explanation in the accompanying text, is diligence information by itself. It doesn't prove the program has stalled, but it's a reasonable prompt to ask why.

Decide Whether You're Looking at a Platform Company or a Single-Asset Pipeline

A platform company's pipeline will show many early-stage programs spread across different indications, all built on one core underlying technology. A single-asset company's pipeline will show far fewer programs, usually concentrated at one stage, because the company's story rests on one candidate.

The funding milestones expected of each type diverge accordingly. A platform company is generally expected to have in vivo data, meaning data from live animal models rather than lab dishes, showing efficacy and safety across more than one indication by the time it raises a Series A round. By Series B, investors typically look for multiple programs backed by in vivo data, with at least one of those programs in the final stages of IND-enabling studies or already at the IND stage. A single-asset company follows a different arc: reaching a development candidate by Series A, and generating actual clinical safety and efficacy data by Series B.

That difference should change what you check. For a platform pipeline, check breadth against real technology validation: does the underlying platform actually produce working candidates across multiple programs, or is the chart mostly placeholders built on the same unproven mechanism? For a single-asset pipeline, check depth: how much real data sits behind the one program carrying the company's valuation, since there's no second asset to fall back on if it fails.

Ask the Right Diligence Question at Each Stage

The questions worth asking change by stage and by what the chart leaves out. A useful starting checklist draws on the factors analysts commonly use to assess a pipeline: the number of assets, the development stage of each one, the target indication, the mechanism of action, trial design and endpoints, regulatory status, competitive differentiation, and partnering potential.

Diligence Questions for Research and Preclinical Programs

For anything labeled research or preclinical, ask what specific milestone places the program at its stated stage. Has the target actually been validated, has a lead compound been optimized, or has a specific IND-enabling study been completed? Ask what in vivo model and species produced the supporting data, since efficacy in one animal model doesn't necessarily translate to another. And if the company points to a proprietary data set or technology moat as the reason to trust an early-stage claim, ask whether that data is independently verifiable rather than simply asserted.

A documented failure mode at this stage is data that is inaccessible or presented without enough context to evaluate the milestone behind it. If a chart entry or deck slide claims a program is "preclinical" without naming the study, the model, or the date, that's a gap worth closing before you rely on the label.

Diligence Questions for Clinical-Stage Programs

For anything in Phase I through Phase III, ask what the primary and secondary endpoints actually are, not just that a trial is "ongoing." Ask which dose cohorts have completed their defined safety monitoring window, what adverse events, if any, have been reported, and when the next data readout is expected. It's also worth checking whether the stated timeline matches the company's own prior guidance, since a slipping timeline that isn't flagged as such is a reasonable prompt for a follow-up question.

The same checklist applies to a cell therapy program running an ascending-dose cohort design, including SMSbiotech's investigational Phase 1b work in COPD. The specific question there is whether each cohort actually completed its monitoring period before the next cohort began dosing, since that sequencing is the mechanism by which early safety signals are supposed to get caught before exposure widens. Asking that question doesn't require or imply any conclusion about whether the therapy works or is appropriate for any patient; it's simply confirming the trial followed its own stated design.

Check Whether Each Asset Is Partnered or Wholly Owned

Pipeline charts often mark partnered assets with a footnote, a partner logo, or a separate column, denoting a licensing or co-development deal rather than work the company is funding and controlling entirely on its own. That distinction changes who pays for the next stage of development and who holds the commercial rights once a therapy reaches approval.

A licensing deal between a smaller biotech and a larger partner typically involves the partner paying an upfront fee plus milestone payments in exchange for exclusive worldwide rights to develop and commercialize a defined set of targets. One documented example of that structure involved a $100 million upfront payment from a major drug maker in exchange for exclusive worldwide licensing rights to several small molecule targets. Once that kind of deal closes, the stage marker shown on the pipeline chart may reflect the partner's development plan and timeline rather than the originating company's own pace, which is why a partnered asset's progress can diverge from what you'd expect based on the smaller company's other wholly owned programs.

Recognize When the Pipeline Chart Doesn't Tell You Enough

When a chart entry shows nothing but a stage label, with no date, no supporting model, and no trial identifier, treat that entry as a placeholder rather than evidence. Cross-check it against the matching clinicaltrials.gov listing, the company's recent SEC filings, or any conference presentation covering the same program. Those sources usually carry the detail the chart itself leaves out.

A program sitting at the same stage marker across more than two reporting cycles without any disclosed reason is worth raising directly with management rather than assuming the worst or the best. And a platform company's failure to disclose an internal pipeline prioritization strategy is a documented failure mode in practice: companies that wait too long to build out an explicit indication strategy often find it complicates fundraising later, once investors start expecting preclinical or clinical data for a defined lead program rather than a broad, undifferentiated list.

Confirm You've Correctly Sized the Program Risk

Before you treat a pipeline chart as reliable enough to act on, run a short completion check against each program you care about. You should be able to state its exact stage milestone, the specific data or filing that supports that stage, whether it is partnered or wholly owned, and the next value-inflection event along with an approximate date.

If any one of those four items is missing, go back to the stage-specific diligence questions above rather than filling the gap with assumption. A pipeline chart read this way lets you rank programs by how close they actually are to their next derisking event, which is a far more useful sorting mechanism than relying on the stage label alone.
